General Policies

  • Co-Pays and deductibles are due at the time of your appointment.
  • We ask you to arrive 10 to 15 minutes prior to your appointment so that your records may be updated.
  • Prescription refill requests will require a 48-hour notice to be sent to your pharmacy.  Please request medication refills through the Patient Portal or by calling the office.
  • All medications taken on chronic basis require regular follow-up visits.
  • All controlled medications require a Urine Drug Screen and a KASPER with each visit.
  • Please do not contact Infinity Family Care providers or staff on social media or by personal cellphone as these avenues are not HIPAA compliant


Most major health insurance plans are accepted including Blue Cross Blue Shield, Medicare, Cigna and others. Health insurance plans change frequently.  Please call our office to confirm if your plan is among those we accept.

Please bring your insurance card to every appointment.

 Co-pays, deductibles, and any expenses not covered by your medical insurance plan are expected at the time of your appointment.  Payment is due upon receipt of your statement. Balances not paid in 90 days will be forwarded to our collection agency for further payment arrangements. 

Self-pay is accepted. Please contact our office for payment arrangements.

Lab testing will be billed by PathGroup. You will receive a separate EOB/statement for those charges.
