Getting Started

Patient appointments are scheduled from 8:00 am to 12:00 pm and 1:00pm to 4:30pm Monday through Friday.  We are closed for lunch each day from 12:00pm to 1:00pm.

If you are unable to keep your appointment or must reschedule, please contact us at 270.512.2512. We kindly ask for 24-hour notice of any cancellation whenever possible.

Please bring any current prescription and/or over-the-counter medication, vitamins, or supplements with you to your appointment.

If you believe your situation is a MEDICAL EMERGENCY, CALL 911 or go immediately to the nearest HOSPITAL EMERGENCY ROOM for treatment. We ask that you contact us as soon as possible thereafter so that we may follow up and assist in your care.


Most major health insurance plans are accepted including Blue Cross Blue Shield, Medicare, Cigna and others. Health insurance plans change frequently.  Please call our office to confirm if your plan is among those we accept.

Please bring your insurance card to every appointment.

 Co-pays, deductibles, and any expenses not covered by your medical insurance plan are expected at the time of your appointment.  Payment is due upon receipt of your statement. Balances not paid in 90 days will be forwarded to our collection agency for further payment arrangements. 

Self-pay is accepted. Please contact our office for payment arrangements.

Lab testing will be billed by PathGroup. You will receive a separate EOB/statement for those charges.
